A Non-Profit Community Child Care Center

Enrolling in our Program 

 

We recommend that before starting your child in our center you and your child visit the center to meet his or her teacher and to familiarize yourselves with the surroundings. We can set up a time for you to visit, just call any day between 8:00 and 4:30 to schedule a visit.

 

The following must be completed and returned at least one week prior to a child’s starting day:

• A physical exam form must be completed and signed by your physician.
  *Children who are of Kindergarten age and older will have a health assessment form which must be filled out by the parent or guardian.

• A certificate of Immunization that is on an Iowa Department of Public Health form.
  *The state of Iowa requires each child have an admission physical exam report. This report includes an immunization record that is in compliance with the Iowa State Health Department. Children who are not up to date on their immunizations will not be allowed to attend the center.

 

The following forms must be filled out completely and signed and dated:

• Personal information

• Medical and dental consent form (for emergencies and illnesses)

• Permission slips for:

  • Field Trips

  • Photographs taken of your children while at the center

• Penalty fee authorization

• Infant/Toddler daily schedule (if applicable)

 It is important that you keep your information current. If at any time during your enrollment at the center you have changes in your enrollment forms please inform the office.
